FUNCTIONAL CAPACITY EVALUATION (FCE) INSTRUCTIONS
A Functional Capacity Evaluation (FCE) is an evaluation of an injured worker's ability to work, and whether or not they can return to work on their regular duty job they were working at the time of their injury, and if they can't, it can determine how much work they can do when they return to work on a different job. An FCE can help the injured worker and their family return to a normal lifestyle again. FCE’s take about 2 hours and it's important to wear work-type clothing, such as, Jeans, Work Slacks, Tennis Shoes or Work Shoes.  Please DO NOT WEAR Dress ClothesOne family member or friend is welcome to observe your FCE if you want them to do so.
It's very important to wear the proper clothing to your PPD evaluation, so please follow the simple guidelines below.

PERMANENT PARTIAL DISABILITY (PPD) EVALUATION INSTRUCTIONS
A Permanent Partial Disability (PPD) evaluation, also called an Impairment Rating Evaluation,
is an evaluation of an injured worker's loss of function of their injured body part following a
job-related or personal injury. The PPD evaluates the "Percent of Loss" of the injured body part by evaluating loss of motion, loss of strength, ligament damage, nerve damage and arthritis that is due to the job injury. The treating physician's diagnosis and the surgical procedures are also used to determine the "Percent of Loss" of the injured body part. Pain
is almost never used to determine the "Percent of Loss" since pain is difficult to objectify.    
INFORMED CONSENT AND QUESTIONNAIRE PACKET INSTRUCTIONS
FOR AN FCE: An Informed Consent and Questionnaire Packet will be mailed to you. Please
Read the Informed Consent, but DO NOT SIGN IT until you arrive for your FCE so we can
witness your signature. Complete the Pain and Disability Questionnaires Before You Arrive
for your FCE and bring the Informed Consent and Questionnaire Packet with you to your FCE.
DO NOT BRING CHILDREN with you for your FCE or PPD Evaluation. Children Are Not Permitted in the Clinic Evaluation Area and THEY CANNOT BE LEFT UNATTENDED in the Waiting Room
Please TURN OFF CELL PHONES before you arrive for your FCE or PPD Evaluation. Cell Phones are very disruptive, so you and your family member or friend are not allowed to take Cell Phone calls in our Center.
PLEASE CALL US with As Much Advance Notice As Possible if you have to cancel your FCE or PPD evaluation so we can schedule someone else in your time-slot. Please call 478-475-9393 between 1-5 pm. 
FOR A PPD: Please Complete and Bring the Questionnaire Packet that was mailed to you for your PPD evaluation. There is No Informed Consent for a PPD Evaluation.

The Blankenship Center is Not Affiliated with any FCE Companies, such as, Key Functional Assessment, Matheson, WorkWell, ErgoScience, Quest
Medical Group or JTECH Medical. The Blankenship WorkEvalTM was Developed by Keith Blankenship and is Owned Exclusively by FCE/PPD Center, Inc.
THIGH AND KNEE INJURIES: Wear or bring loose fitting shorts that can be pulled up to your Groin so your Mid-Thigh can be measured accurately, as the top left picture shows.
LOWER LEG, ANKLE AND FOOT INJURIES: Wear or bring shorts or loose fitting pants
that can be pulled up to your Knee so your Calf can be measured accurately.
NECK AND SHOULDER INJURIES: Wear a Short Sleeve or Sleeveless top so your Neck and/or Shoulder and Upper Arm can be examined properly.
LOW BACK, PELVIS AND HIP INJURIES: Wear or bring loose fitting shorts that can be pulled up to your Groin so your Mid-Thigh can be measured accurately as shown in the top left picture. 
DRIVING SOUTH ON 1-75: Take Exit 169 Arkwright Road
off I-75, and Turn Right at the bottom of the Exit Ramp and go 1 Block and Turn Left at the Stop Light, or South, onto Riverside Drive, and go exactly 1.1 miles and then 
Turn Right into theRiverside North Office Park just past the Five Star Chrysler, Dodge, Jeep, Mazda Dealership,
and just across from the Red Oak Drive Overpass on I-75.
Then proceed to the back of the Office Park and park in front of Suite C-5 in Building C.
FROM THE ARKWRIGHT ROAD EXIT OFF 1-75
DRIVING NORTH ON 1-75: Take Exit 167 Pierce Ave, Turn Right, or North, on Riverside Drive and go exactly 1.2 miles and Turn Left into theRiverside North Office Park just before the Five Star Mazda Dealership. and
just across from the Red Oak Drive Overpass on I-75. Then proceed to the back of the Office Park and park
in front of Suite C-5 in Building C.
FROM THE PIERCE AVE. EXIT OFF 1-75
The Blankenship FCE/PPD Center is on Riverside Drive in the RIVERSIDE NORTH OFFICE PARK, in back of the Five Star Auto Dealership. Turn into the Riverside North Office Park by Five Star Mazda, just across from the RED OAK DRIVE Overpass on I-75, and The Blankenship FCE/PPD Center is in Building C, Suite C-5, which is at the Back-Right-Corner of the Parking Lot of the Riverside North Office Park.

DRIVING SOUTH ON 1-75: If you are driving South on
1-75 and you missed Exit 169 Arkwright Road, then proceed to Exit 167 Riverside Drive To Pierce Ave,
and Turn Right, or North, on Riverside Drive and go exactly 0.8 of a mile and Turn Left into the Riverside North Office Park just before the Five Star Mazda Dealership, and just across from the Red Oak Drive Overpass on I-75, and then proceed to the back of
the Office Park and park in front of Suite C-5 in
Building C.
DRIVING NORTH ON 1-75: If you are drving North on 1-75 and you missed Exit 167 Pierce Ave, then proceed to
